Logbook

All trainees must submit a log book as prescribed by the College as documentation of work done during their tenure in the ICU. This must strictly be in the prescribed format. Templates, examples and instructions are available and can be downloaded from this site’s download section.

 

  • The logbook (duly signed by the Course Director/ teacher and prepared STRICTLY as per the specifications mentioned) must be submitted by the candidate so as to reach the college latest by 30th November (for Part 1 candidates) and by 30 September (for Part 2 candidates) before the due examination.
  • Under exceptional circumstances, logbooks will be accepted for a further period of 7 days from the above deadline provided they are submitted with valid reason for delay, signed by the medical director of the institute along with a late submission fee of Rs. 10,000/- (see Calendar of College of Critical Care Medicine for details) by DD/ multi-city cheque in favor of “College of Critical Care Medicine” and payable at Mumbai.
  • Failure to submit logbook in time automatically disqualifies candidate from appearing for the coming examination. These candidates will have to re-register for the future examination all over again. (No exception will be made to this rule except for candidates who are repeaters and had submitted/passed their logbooks in the previous examination)
  • Logbooks carry a 10% weight-age in the whole examination. Passing the logbook is a pre-requirement for being allowed to take rest of the examination. Failure to get at-least 50% marks in logbook will result in candidate being declared fail for the whole examination and they will not be entitled to appear in the coming examination. Such candidates will have to resubmit and pass their logbook and appear in the Repeat examination.
